These forms must be submitted to the District. They deal with negotiated terms from the CBA. Please check the notes on the proper submission of each.
If a member does not wish to participate in the sick leave bank, they must complete an opt-out form and provide it to the payroll supervisor no later than Sept. 15th. If a bargaining unit member does not have at least one (1) day of sick leave, the District shall advance the member one (1) day in order for the member to become enrolled in the sick leave bank.
To utilize the Sick Leave Bank, please fill out this form and submit to uaeamembers1@gmail.com. This will be submitted on your behalf to the district.
Personal Leave can be submitted through Aesop unless the leave is on a Restricted Day as defined by the Collective Bargaining Unit. To request Restricted Personal Leave,
You must request and submit your application electronically 20 days in advance to the Mincy Center,
You must be in the first five percent of your bargaining unit to request the day(s), and
You may not submit more than two requests per school year.
Failure to follow the aforementioned criteria will result in a denial of request(s) for restricted personal leave. Leave will be granted and entered for you in Aesop regardless of the reason for leave application.
Per the CBA, notwithstanding the limit of 5% of the bargaining unit, Restricted Personal Leave for a qualifying life event will still be granted.
Note: f your personal leave is denied or you have no more personal days, you may still take the day off, but it will be unpaid.
To apply for unpaid personal leave, download this form and submit it to Human Resources. Written application for non-paid personal leave should normally be made one (1) week in advance to the Chief Talent Officer with the principal's or supervisor's recommendation. The staff member will be notified within forty-eight (48) hours of approval or disapproval after the Chief Talent Officer receives the request. If applications are not approved, the staff member will have the opportunity to meet with the principal, supervisor, Chief Talent Officer and/or the Superintendent to review the request.
If you are asked to serve as a substitute teacher to cover classes outside of your normal scope of duties, you may be eligible for additional pay.

Some funds are made available to UAEA for distribution to bargaining unit members. Each of these funds are disbursed from the UAEA Treasurer.
UAEA members are eligible up to $200 for conference/workshop fees for professional development (non-credit). A total of $9,000 is available annually for distribution 3 times annually. Funding must be requested and approved in advance of the event.

There are additional free-credit opportunities through Fee Waivers.
These waivers applies to graduate level tuition at local university partners.
Applications open and close throughout the year and notification is sent via email to all staff.
